Who we service and how the transition works

  • Grandfathered Participants – If you were assessed eligible for a Home Care Package or were already receiving one on or by 12 September 2024, then you fall into this category. Your funding level will carry over into the Support at Home program, and your fee or contribution arrangements will not increase because of the reform.

  • New Entrants – If you were assessed for a Home Care Package after 12 September 2024 (or you will be assessed under the Support at Home program after full rollout), then you’re treated under the reformed model. Your budget, service classification and contributions will follow the new Support at Home rules (including the new service categories and contribution framework).

Important changes under the Support at Home model

  • Services are now categorised into Clinical, Independence, and Everyday Living supports.

  • Funding is provided as a quarterly budget rather than the prior daily subsidy model.

  • Participant contributions for Independence and Everyday Living supports will vary based on income/assets.

  • If you are grandfathered, you keep your existing arrangements; if you are a new entrant, you will adopt the new approach.
